What is Botulinum Toxin?

The term “botulinum toxin” covers various, similar neurotoxic proteins that are produced by the Clostridium botulinum bacterium. In science, a distinction is made between botulinum toxin type A to type G.
Botulinum toxin is a muscle relaxant. It inhibits the release of acetylcholine, which is required to trigger muscle contraction. Limiting or even preventing the transmission of impulses from nerve to muscle leads to a partial or complete inhibition of the muscles.

The commonly distributed toxin can be found in multiple variety. In specialist circles, these are known as Botulinum A-G, where the use Botulinum A is primarily used for medicinal purposes. Most people will have heard of this in connection with aesthetic medicine by the name.

It has been although used in many different medical fields since the last century. First it was used in the field of neurology to counteract movement disorders. Today it is commonly found in therapies against migraines and excessive sweating (hyperhidrosis) as well as the treatment of various tremors (involuntary muscle spasms) and cramps.
Due to its muscle relaxing effects, botulinum toxin has found its way into the aesthetic medicine throughout the past decade, where it is used to tighten wrinkles.

The active agent will be degraded by the body's own functions and so the effects are only temporary. How long it takes for the effects to wear off differs from individual to individual depending on where the botulinum toxin is injected and how fast the active agent is degraded.

Botulinum Toxin against Hyperhidrosis

Hyperhidrosis is considered as the excessive production of sweat. It occurs unrelated to temperature, physical strain or season. Some people only sweat profusely in specific areas, others may experience an enhanced sweat production throughout the entire body.

Besides a laser, the hyperhidrosis therapy can also be accompanied by botulinum toxin injections. The botulinum toxin will be injected into the affected area, in order to block the nerve impulses which are controlling the sweat production.

Botulinum toxin for smoothing wrinkles

Botulinum toxin treatment can be used to smooth the kind of wrinkles caused by recurring muscle movements. For this purpose, very small, weakly concentrated amounts of botulinum toxin are injected into the muscles to be treated. This specifically inhibits the muscle pull, which results in a smoothing of the skin area.

The drug is mainly used for the treatment of mimic wrinkles on the face (e.g. frown lines, mouth wrinkles), neck and décolleté. The following types of wrinkles/problem areas, among others, can be treated with the muscle relaxant:
  • Frown lines/glabellar wrinkles: Treatment of vertical wrinkles between the eyebrows to achieve a more relaxed and youthful appearance.
  • Thinking forehead/worry lines: Reduction of horizontal frown lines caused by persistent frowning.
  • Eyebrow lift: lifting and accentuating the eyebrows to achieve an open look and a more youthful appearance.
  • Crow's feet/laugh lines: Reduction of fine lines and wrinkles around the outer corners of the eyes caused by smiling and facial expressions.
  • Marionette lines/Mentolabial folds: Smoothing wrinkles between the corners of the mouth and chin to create a harmonious appearance in the lower facial area.
  • Bunny Lines (nose wrinkles): Treatment of fine lines on the sides of the nose that appear when laughing or frowning.
  • Mouth wrinkles/pleated lines: Reduction of vertical lines and wrinkles around the mouth area to achieve a smoother and younger appearance.
  • Strawberry chin/cobblestone chin/chin wrinkles: Smoothing wrinkles or dimples around the chin to achieve a more even appearance.
  • Smoker's lines: Treatment of fine lines and wrinkles around the mouth that can result from smoking.
  • Gum smile (Gummy smile): Correction of a smiling facial expression where the gums are too visible.
  • Facial contouring and narrowing: altering the shape of the face to define contours or reduce certain areas.
  • Migraine Botulinum Treatment: use of botulinum toxin to treat migraines and relieve associated symptoms.
  • Lip Flip (for fuller lips without fillers): Lifting the lip contour to create the appearance of fuller lips without using fillers.

  • Botulinum toxin for sweating (hyperhidrosis): Treatment of excessive sweating through the targeted application of botulinum toxin to the affected areas.

  • Bruxism/teeth grinding (Masticatory muscle): Relaxation of the masticatory muscles to reduce teeth grinding and tension in the jaw area.

  • Nose narrowing: use of Botulinum toxin or filler to relax the nasal muscles and lift the tip of the nose to visually narrow the nose.

  • Migraine and depression: use of botulinum toxin to relieve migraine attacks as well as to help treat depression.

  • Mouth corner elevation: Raising the corners of the mouth to create a friendlier and less tired facial expression.

  • Tension neck and shoulder muscles: relaxing the muscles in the neck and shoulder area to relieve tension and pain.

  • Nefertiti lift (Turkey neck/platysma wrinkles): Tightening and smoothing the skin on the neck area for a more youthful appearance and a clearer chin line.

Contraindication for a Botulinum Toxin Treatment

Even though the professional use of botulinum toxin injections will only seldom cause risk or side effects, there are reasons to forgo a treatment. The attending person should enlighten you about these risks during your first consultation. Inform your doctor about your medical history as well as the possible intake of medication beforehand, in order for us to consult you in the best possible way.

The contraindications for a botulinum toxin therapy especially include:

  • Pregnancy and Nursing Period: Non-medical procedures should always be postponed during a pregnancy or the nursing period.
  • Infected Treatment Area: Any infections in the area which is meant to be treated, should first be fully healed before the begin of the procedure.
  • Illness: If weakness of the muscles caused by illness such as myasthenia gravis is acute, the treatment should be postponed.
  • Intake of Medicine: Injections should be postponed if you are currently on or have just stopped the intake of certain antibiotics (aminoglycoside antibiotics). Medication which contains acetylsalicylic acid (ASS) should as well not be taken beforehand.

Process of Wrinkle Treatment using Botulinum Toxin

Before you start the actual therapy, you should definitely always conduct a consultation in the practice of choice, which will go over your medical history as well as your momentary intake of medicine and involve an examination of the skin area in question. Furthermore, you should have the opportunity to put forth your expectation for the treatment.

This varies from patient to patient and depends on physical conditions and the treated body region.

A follow up appointment to monitor even progress should be arranged 14 days after the initial treatment. During which specific areas can be reinjected if this is deemed necessary.

Post Treatment Behavior Advice

  • 24 Hour Rest Period: After the botulinum toxin treatment, you should NOT lay down for three to four hours and give your skin and entire day of rest and time for recovery. This means: no physical activities for the rest of the day after the treatment.
  • No Facial Massages: After the treatment you should go without massages to the injected area. This may cause the botulinum toxin to disperse into unwanted areas and alter the wanted effect.
  • No Peelings for 14 Days: After the treatment you should go without chemical peelings or laser treatments for the following two weeks. You can although apply your usual facial care.

Risks and Side Effects of a Botulinum Toxin Treatment

Professionally conducted, treatments using botulinum toxin harbor hardly any side effects:


  • Swelling and redness: Slight swelling and redness may occur at the injection site. However, these are temporary and will resolve on their own.
  • Undesirable distribution: In rare cases, botulinum toxin may also be distributed to neighboring muscle groups, temporarily causing undesirable side effects there.
  • Bruising: If vessels are hit during the injection, bruising may occur in the injection area. However, the use of extremely thin needles means that bruising hardly ever occurs.
  • Bruising due to medication: Certain blood-thinning medications can prolong the bleeding time and thus increase the risk of bruising. Therefore, it is important that you inform your doctor about any medications you may be taking during your consultation.
  • Allergic reactions: Allergic reactions to botulinum toxin are very rare.

Although, unwanted side effects may also occur due to false or poor treatment execution.

If, for example, too much active agent is injected, the communication between muscles and nerves can be hindered completely. This can cause weakness of muscles, or in case of a treatment of platysma lines in the neck area, lead to difficulties in swallowing. The cause of this does although not, as commonly perceived, in the active agent, but in the problematic implantation of the treatment.

Advantages and Disadvantages of a Botulinum Toxin

Treatment A botulinum toxin injection refers to an anti-aging treatment, which is minimally invasive and can be conducted with little effort or restrictions. This type of wrinkle reduction, furthermore, is low in risk, offers a quickly visible result and leads to no downtime. The body will also break down the botulinum toxin over time. This fact also presents its largest disadvantage of this active agent of having to repeat the treatment in regular periods in order to maintain the results.

What else is there to know about botulinum toxin in the face?

Before a treatment with botulinum toxin, there is often the concern that restricted facial expressions will occur. This risk does not exist with treatment by an experienced specialist. The amount of botulinum toxin used is so small that permanent restrictions and damage are extremely unlikely. 

If there is hypersensitivity to botulinum toxin or other components of the solution, treatment should be avoided. Uncertainties such as these will be discussed in a detailed consultation prior to treatment.

When one stops regular treatment with botulinum toxin, there is a decrease in the effects achieved. The substance is completely broken down by the body and the muscle regains its full flexibility. For this reason, it is advisable to repeat the treatment regularly. 

After a botulinum toxin treatment on the face, the effect is usually not immediately apparent. It can take several days. The treatment itself does not cause any pain. The sting is briefly uncomfortable, but this passes quickly.

It may happen that the botulinum toxin does not work. However, this is extremely rare and affects very few people. These are virtually immune to botulinum toxin because they have antibodies that bind the substance and make it ineffective.

Botulinum toxin A is obtained from a natural bacterium. The toxin inhibits the messenger substances between the muscles and the nerve. After the injection, the nerve can no longer tell the muscle to contract. Thus, it can no longer provide the traction that forms wrinkles on the surface.
